Qualified candidates include those with a high school diploma or GED plus health-care experience, an associate degree with clinical experience, an LPN/LVN license in TX, or CHW certification.

Qualifications

• Must possess one of the following: High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population within a clinical health care role
• Associate's degree with 3+ years of clinical experience
• Unrestricted LPN/LVN license in the state of TX with 3+ years of clinical experience
• CHW certification with 3+ years of clinical experience
• Intermediate level of proficiency with Microsoft Office, including Word, Excel, and Outlook
• Access to reliable transportation and the ability to travel up to 75% of the time in this ‘assigned area’ to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
• High speed internet in place of residence

Responsabilités

• Complete face-to-face assessments in members’ homes to address unmet needs
• Travel up to 75% of the time, including travel up to 100 miles from the assigned work location, to meet business needs
• Meet documentation and turnaround times based on state contractual requirements
• Time management and organizational skills
• Ability to create, edit, save and send documents, spreadsheets and emails
• Reside within commutable distance of assigned duties
• Works on Service Plan
