Qualifications

• Face-to-face assessments in members' homes.
• Travel up to 75% of the time, including up to 100 miles from the assigned work location.
• Meet documentation and turnaround times based on state contractual requirements.
• Ability to create, edit, save and send documents, spreadsheets and emails.
• Reside within commutable distance of assigned duties.
• Works on Service Plan.

Responsibilities

• Complete face-to-face assessments in members’ homes to address unmet needs.
• Travel up to 75% of the time to meet business needs and visit members in homes or other settings.
• Ensure documentation meets state contract requirements and timelines.
• Manage time effectively and stay organized while serving members.
• Create and share documents, spreadsheets, and emails as part of service delivery.
• Maintain commuting distance to assigned duties and operate within the assigned area.
