UnitedHealthcare is seeking a Field Based HSS Clinical Coordinator RN in the Bexar County, TX area. The role blends field work with remote tasks and requires travel to member homes and facilities.

You will assess care needs, coordinate services, and communicate with care teams to ensure high-quality outcomes while navigating Medicaid/HCBS contexts. Telecommuting is permitted under policy.

Competenze

• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• Reliable transportation, a valid driver, and ability to travel within assigned territory.
• High-speed internet at residence.

Mansioni

• Assess, plan and implement care strategies individualized by patient and directed toward appropriate level of care.
• Identify and initiate referrals for social service programs including financial, psychosocial, community and state services.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate with stakeholders the required health information to ensure coordinated care.
• Advocate for patients and families to ensure needs are represented by the health care team.
• Support performance with development opportunities and clear direction.
