SteveRecruiter, LLC is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This RN will perform skilled visits, assess patients, develop plans of care, and coordinate with physicians and the care team.

The ideal candidate will hold an active Texas RN license, have recent home health case management experience, strong documentation habits, and be able to travel locally.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Knowledge of Medicare home health, OASIS, plan-of-care management.
• Ability to work independently with strong communication.
• Willingness to travel locally within Fort Worth / Tarrant County.
• EMR experience preferred.

Responsabilités

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the team.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
