Professional Recruiting Consultants, Inc. is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and surrounding Tarrant County.

The ideal candidate will manage an assigned caseload, perform skilled nursing visits, and collaborate with physicians, patients, families, and the care team. Active Texas RN license and recent home health case management experience are required; EMR experience is preferred.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management.
• Ability to travel locally throughout Fort Worth / Tarrant County.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates.
• Provide patient and caregiver education on disease management, medications, safety, and care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
