Job summary

Professional Recruiting Consultants, Inc. is seeking an experienced Home Health RN Case Manager for field-based duties across the Dallas–Fort Worth area, including Fort Worth and Tarrant County. The role requires an active Texas RN license and recent home health case management experience.

You will perform skilled nursing visits, develop plans of care with physicians and families, complete OASIS and visit notes, educate patients, and ensure regulatory compliance while traveling locally.

Qualifications

Active Texas RN license in good standing is required.
Recent Home Health RN Case Management experience required.
Understanding of Medicare home health, OASIS, and plan-of-care management is essential.
Ability to work independently while maintaining strong communication with the clinical team.
Strong clinical judgment, organization, documentation and time-management skills.
Valid driver's license and ability to travel locally throughout Fort Worth / Tarrant County.
EMR experience preferred.

Responsibilities

Manage an assigned home health patient caseload in the field.
Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
Complete accurate clinical documentation, including OASIS and visit notes.
Monitor patient progress and communicate updates to the team.
Educate patients and caregivers on disease management, medications, safety, and care goals.
Coordinate care with therapy, social work, aides, physicians, and leadership.
Maintain compliance with home health regulatory, documentation, and quality standards.
