Job summary

Aspire Talent Solutions is seeking an experienced Home Health RN Case Manager to serve Fort Worth, TX and the surrounding Tarrant County area. This field-based role requires strong assessment skills, independent patient management, and meticulous documentation.

The ideal candidate will hold an active Texas RN license, recent home health case management experience, and proficient knowledge of OASIS, plan-of-care, and Medicare home health regulations. Travel locally is expected.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health, OASIS, plan-of-care management.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
