Job summary

ExcelliSearch, Inc. is seeking an experienced Home Health RN Case Manager for field-based care serving Fort Worth, TX and the surrounding Tarrant County area. The role requires a clinically strong RN with recent home health case management; you will manage a caseload in the field.

Responsibilities include skilled visits, care planning with physicians and families, and timely OASIS documentation. Travel locally is required, with competitive full-time compensation and benefits.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the clinical team.
• Provide patient and caregiver education on disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership; maintain regulatory compliance.
