Job summary

H. T. PROF Group is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and surrounding Tarrant County.

This full-time position requires a clinically strong RN with recent home health case management experience, excellent assessment and documentation skills, and the ability to manage an assigned patient caseload independently in the field. You will travel locally to patients' homes, coordinate with physicians, families, and a multidisciplinary team,

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
