You will coordinate with physicians, families, and the interdisciplinary team, ensure timely documentation, and maintain compliance with Medicare home

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health and OASIS.
• Ability to travel locally within Fort Worth/Tarrant County.
• Proficient in clinical documentation and care coordination.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, and recertifications.
• Develop, update, and coordinate plans of care with physicians, patients, families, and the care team.
• Complete accurate documentation including OASIS and visit notes.
• Monitor patient progress and communicate updates to the care team.
• Educate patients and caregivers on disease management, medications, and safety.
• Coordinate with therapy, social work, and office leadership.
• Maintain compliance with home health regulations and quality standards.
