Job summary

SteveRecruiter, LLC is seeking a Home Health RN Case Manager for a field-based role serving Dallas and Fort Worth areas. This position is best suited for a clinically strong RN with recent home health case management experience who can independently manage a patient caseload in the field.

Requirements include an active Texas RN license, knowledge of Medicare home health, OASIS, plan-of-care management, and strong documentation.

Qualifications

• Recent home health RN case management experience required.
• Active Texas RN license in good standing.
• Strong understanding of Medicare home health, OASIS and plan-of-care management.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the team.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
