Ethos is seeking an experienced RN Home Health Case Manager for a field-based role serving Fort Worth, TX. You will manage a patient caseload, perform skilled nursing visits, and develop individualized care plans in collaboration with physicians and families.

The ideal candidate has recent home health case management experience, strong assessment skills, and documentation habits. This role offers excellent benefits and relocation assistance where applicable.

Qualifications

• Active RN license
• Recent home health case management experience
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing
• Ability to work independently with strong communication with the clinical team

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update and coordinate individualized plans of care with physicians, patients, families and the interdisciplinary team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
