Job summary

CVS Health is seeking a Field Case Management Coordinator in Kentucky to support members across Jefferson County. The role requires in-state travel 80-90%, a Bachelor’s or Master’s in behavioral health, and a willingness to visit members in home, school, and clinical settings.

The coordinator will assess needs, coordinate care plans, and advocate for services while partnering with care teams to improve health outcomes. 40 hours/week, full-time.

Qualifications

• Minimum 2 years of experience in behavioral health, social services, psychology, sociology, marriage and family therapy, or counseling.
• Minimum 1 year of experience in MS Office Suite applications, including Word and Excel.
• Willing and able to travel 80-90% of their time to meet members face to face in their assigned region.
• Candidate must reside in Jefferson County, KY.

Responsibilities

• Evaluate members' benefit plans and available internal and external programs/services to resolve care needs.
• Identify high-risk factors and provide proactive support interventions.
• Coordinate Wellness care plan activities and monitor member care needs.
• Utilize case management processes in compliance with regulatory and accreditation guidelines.
