CVS Health is seeking a dedicated Case Management Coordinator to work remotely within Ohio, with up to 50% travel to members' homes and related sites. This full-time role focuses on coordinating behavioral health care for children and families, engaging with multiple care teams across regions, and ensuring timely, effective services.

Applicants should have a bachelor’s or non-licensed master’s in behavioral health or related field, plus 2+ years of relevant experience.

Formación

• Bachelor’s degree or non-licensed master level clinician.
• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related field.
• 2+ years in family systems, case management, or child development.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing to travel within the region up to 50% and may travel to New Albany office for trainings.

Responsabilidades

• Provide care coordination, support, and education to members.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with teams to improve quality of care and outcomes.
• Utilize care management tools and data to assess needs and eligibility.
• Engage with members to empower informed healthcare decisions.
• Act as single point of contact to remediate gaps in care.
