CVS Health's Aetna Better Health is hiring a Case Management Coordinator in Kentucky to support care management for members, advocating, coordinating resources and educating families. This field role requires 80-90% in-state travel with meetings at the Louisville office and member homes as needed.

Qualified candidates must reside in Jefferson County and have a Bachelor’s or Master’s degree in behavioral health or a related field, plus strong MS Office skills and trauma-informed care experience.

Qualifications

• Bachelor's or Master's in behavioral health or related field required.
• Experience in behavioral health, social work or counseling preferred.
• Travel 80-90% in the assigned region.

Responsibilities

• Assess and coordinate care to improve member health outcomes.
• Advocate, educate and support members and families.
• Coordinate wellness plans and monitor care needs.
• Document care activities in accordance with guidelines.
