Job summary

CVS Health is hiring a Population Health/Wellness Case Management Coordinator to support care management for members. The role is a telework position with up to 50% travel within the state, including Louisville meetings and training.

You will collaborate with case managers, Medical Directors, and health programs to optimize outcomes for Kentucky SKY populations. The position requires a Bachelor's degree in behavioral health or related field, 2 years’ experience in behavioral health, and

Qualifications

Experience and knowledge of Foster Care, juvenile justice systems, Adoption Assistance, and related behavioral health services in Kentucky SKY populations.
Minimum 2 years’ experience in behavioral health or related field.
Proficient in MS Office (Word, Excel) and general computer literacy.
Flexibility to work beyond core business hours (Mon-Fri 8am-5pm) to serve children and families.

Responsibilities

Evaluate member benefit plans and programs; identify high-risk factors and initiate care interventions.
Consult with case managers, Medical Directors, and health programs to overcome barriers and improve outcomes.
Document and monitor care using case management and quality processes in compliance with guidelines.
