Job summary

CVS Health is seeking a Care Management Coordinator to join a field-based team in Kentucky. The role requires 80-90% travel within the Salt River Trail Region and across listed counties; candidates must reside in Kentucky.

As a CMC, you will conduct comprehensive member assessments, coordinate care plans, and collaborate with state and behavioral health partners to improve health outcomes while navigating regulatory guidelines and driving patient advocacy.

Qualifications

• 2 years' experience in behavioral health, social services, or human services field.
• 1+ year(s) of experience with Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention in KY SKY populations.
• 1 year experience with MS Office Suite applications, including Word and Excel.

Responsibilities

• Evaluation of Members: conducts comprehensive assessment of member's needs/eligibility and recommends an approach; identifies high risk factors.
• Coordinates and implements assigned care plan activities and monitors progress to support health and compliance with juvenile justice guidelines.
• Collaborates with state and behavioral health partners to ensure members understand their health care needs.
