Job summary

CVS Health, including Aetna Better Health, is seeking a Case Management Coordinator in Kentucky to support care management for members. The role requires 80-90% in‑state travel, with travel to the Louisville office for meetings and training.

A Bachelor’s or Master’s in behavioral health or related field is required, along with 2+ years in behavioral health, and 6+ months in foster care settings. 40 hours/week, full‑time.

Qualifications

• Minimum 6 months experience in Foster Care or related systems for behavioral health services.
• Minimum 2 years of experience in behavioral health, social services, psychology or counseling.
• Proficiency with MS Word and Excel.

Responsibilities

• Evaluate member needs and coordinate care plans to improve health outcomes.
• Identify high-risk factors and provide proactive support for member needs.
• Coordinate wellness activities and monitor ongoing care.
• Collaborate with case managers and medical directors to resolve barriers.
