Qualifications

• Minimum 2 years in behavioral health, social services, psychology, or counseling.
• Minimum 1 year of MS Office experience (Word/Excel).
• Willing to travel 80-90% to meet members in the assigned region.
• Residence in Jefferson County, KY.

Responsibilities

• Evaluate member benefit plans and available programs to improve health outcomes.
• Identify high risk factors and coordinate proactive support interventions.
• Coordinate Wellness care plans and monitor member needs.
• Consult with case managers and professionals to address care barriers and promote engagement.
• Document care in line with regulatory and company policies.
