Qualifications
- 2 years' experience in behavioral health, social services, or human services field.
- 1+ year(s) of experience with Foster Care and juvenile justice systems, Adoption Assistance, trauma-informed care, and related practices.
- 1 year experience with MS Office Suite applications, including Word and Excel.
Responsibilities
- Evaluate members and assess needs, eligibility, and plan options.
- Coordinate and monitor care plans to support health and wellbeing.
- Collaborate with state and behavioral health partners to align care needs.
- Identify quality of care issues and escalate as needed.
- Provide advocacy, coaching, and education to members and families.
