This telework position is field-based with 80-90% travel across Salt River Trail Region counties, requiring a valid KY driver's license and reliable transportation.

You will evaluate member needs, coordinate care plans, collaborate with health partners, and promote evidence-based practices while adhering to CVS Health and HIPAA guidelines.

Qualifications

• 2 years' experience in behavioral health, social services, or human services field.
• 1+ year(s) of experience with Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention.
• 1 year experience with MS Office Suite applications, including Word and Excel.
• Must have reliable transportation and travel 80-90% of the time in Salt River Trail Service region.

Responsibilities

• Evaluation of Members: assesses needs/eligibility and recommends an approach to case resolution.
• Coordinates and implements care plan activities and monitors progress.
• Collaborates with state and behavioral health partners to ensure member understanding of health care needs.
• Ensures quality of care and compliance with regulatory guidelines and company policies.
