Job summary

Addison Group is seeking a Community Partnerships Liaison in Philadelphia, PA. The role focuses on field-based outreach to build and manage referral partnerships with physicians, discharge planners, case managers, and community facilities, driving growth and visibility in the region.

You will spend significant time in the community, delivering presentations and events, and coordinating with Intake, Scheduling, Marketing, and Operations to ensure a smooth start of care.

• 1–3 years of sales or sales-adjacent experience.
• Track record of meeting goals and building strong relationships.
• Strong communication skills with a wide variety of partners.

Responsibilities

• Own referral development for an assigned territory and generate qualified leads.
• Build relationships with physicians, discharge planners, case managers, facilities, senior living communities, and other referral partners.
• Spend the majority of time in the field meeting partners and representing the organization in the community.
• Deliver partner presentations, in-services, and community events to communicate services and value.
• Create a weekly territory plan with the Director of Growth.
• Track outreach and marketing activity to show which relationships drive results.
• Ensure smooth referral through start of care with Intake, Scheduling, Marketing, and Operations.
• Keep Salesforce and tracking tools up to date with activity, leads, and next steps.
