Job summary

Addison Group is seeking a Community Partnerships Liaison in Harrisburg, PA. This field-focused role emphasizes building strong referral networks, meeting physicians, discharge planners, and community partners, while driving growth and visibility for the organization.

You will spend substantial time in the field, deliver presentations, and collaborate with Intake, Scheduling, Marketing, and Operations to ensure a smooth start of care.

• 1–3 years of sales or sales-adjacent experience.

Responsibilities

• Own referral development for an assigned territory, generating qualified leads and supporting client-start and growth goals.
• Build relationships with physicians, discharge planners, case managers, facilities, and other referral partners.
• Spend majority of time in the field meeting partners and representing the organization in the community.
• Deliver partner presentations, in-services, and events communicating services and value.
• Create weekly territory plan with Director of Growth prioritizing high-potential partners.
• Identify new referral sources and local opportunities to expand visibility.
• Keep Salesforce and tracking tools up to date with activity, leads, and next steps.
• Review pipeline and results with Director of Growth and adjust strategy.
