Job summary

Qualifications

• Ability to travel 70% of the time.
• Access to reliable transportation.
• 3-5 years' relevant work experience.
• 1-2 years’ experience providing clinical support services.
• 1 year of community outreach experience and knowledge of LA resources.
• 1 year experience with clients with clinical needs.

Responsibilities

• Conduct home visits to reinforce care plans and reduce hospitalizations.
• Educate providers on social determinants affecting care.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Identify and address cultural and socio-economic barriers to care.
• Assist with care plan implementation and documentation in EHR.
• Travel extensively to client homes and community locations.
