Job summary

CentroMed in San Antonio, TX is seeking an outreach-focused professional to identify potential clients for health services and inform the public about available programs. Under supervision, you will lead aggressive field outreach rather than office visits.

Duties include community outreach at homes, schools, churches, and health fairs; follow-up with clients by phone, mail, and in person; posting informational materials; staying informed about services; perform other duties as assigned.

Qualifications

• Education: High school diploma or equivalent.
• Experience: Outreach, customer relations and/or community based programs; one-to-one interactions with clients.
• Bilingual English/Spanish preferred.

Responsibilities

• Performs outreach in the community at homes, schools, churches, agency sites and at health fairs.
• Follow up with clients through phone, mail, and in person; maintain documentation on clients and activities.
• Post informational posters, flyers and brochures at local stores, community centers, schools and churches.
• Maintains knowledge of programs and services available to clients/patients.
• Performs other duties as assigned.
