Job summary

WellPower in Colorado is seeking Clinical Case Managers to deliver comprehensive psychological and behavioral case management within a continuum of services designed to improve or maintain clients’ ability to function in the community.

This field-based role involves regular visits to jails, courts, hospitals and client residences, with transportation in your own vehicle and mileage reimbursement; a valid Colorado driver’s license is required.

Qualifications

• Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or other related human services degree.
• One year experience in mental health or human services preferred.

Responsibilities

• Assess and monitor clients toward recovery.
• Coordinate care with a multidisciplinary team.
• Provide transportation to clients for appointments and community resources.
• Complete all clinical documentation of consumer interaction.
• Attend court, jail, hospital, and community agency engagements.
• Maintain HIPAA compliance and client confidentiality.
