Job summary

True Mental Health Services seeks a Community Case Manager to provide comprehensive case management for individuals facing mental health and behavioral health challenges in home and community settings.

You will assess needs, connect to resources, coordinate care with families, schools, and providers, and advocate for essential services while maintaining timely documentation. Field-based role with travel across communities.

Qualifications

• Bachelor’s degree in psychology, social work, medicine, nursing, rehabilitation counseling, sociology or related behavioral health field.
• Valid Texas driver’s license with acceptable driving history.
• Reliable transportation with proof of insurance.

Responsibilities

• Conduct service needs assessments and ongoing client evaluations.
• Develop, implement, and review individualized service plans.
• Connect clients and families with community resources and support services.
• Coordinate care with providers, families, schools, medical professionals, and community agencies.
• Advocate for clients to obtain needed services and benefits.
• Monitor client progress and adjust services as needed; assist with discharge planning and aftercare coordination.
• Maintain accurate clinical documentation and case notes within required timeframes.
• Participate in daily team meetings and case reviews.
• Complete chart audits and ensure Medicaid compliance standards are met.
• Serve as a liaison between clients, families, referral sources, and providers.
• Support clients with psychiatric, substance use, or crisis-related challenges.
