Job summary

True Mental Health Services is seeking a Community Case Manager to support individuals in accessing mental health and community resources. The role involves comprehensive case management in home and community settings, travel throughout the community, and collaboration with families, schools, and providers.

The ideal candidate holds a Bachelor’s degree in a behavioral health field, has a Texas driver’s license, and is committed to compassionate care for diverse populations.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, or a related behavioral health field (per ad).
• Experience or ability to work in community settings and with diverse populations is preferred.

Responsibilities

• Conduct service needs assessments and ongoing client evaluations
• Develop, implement, and review individualized service plans
• Connect clients and families with community resources and support services
• Coordinate care with providers, families, schools, medical professionals, and community agencies
• Advocate for clients to obtain needed services and benefits
• Monitor client progress and adjust services as needed
• Assist with discharge planning and aftercare coordination
• Maintain accurate clinical documentation and case notes within required timeframes
• Participate in daily team meetings and case reviews
• Complete chart audits and ensure Medicaid compliance standards are met
• Serve as a liaison between clients, families, referral sources, and providers
• Support clients experiencing psychiatric, substance use, or crisis-related challenges
• Maintain professionalism and compassionate care in all interactions
