Job summary

CVS Health Corporation is seeking a Clinical Case Manager Behavioral Health to perform in-person assessments and craft individualized care plans for Medicare/Medicaid Waiver members. You will coordinate services across providers, document activities, and participate in interdisciplinary teams while advancing professional growth.

The role requires an active IL Behavioral Health license (LCSW/LCPC/LMFT), a Master’s degree in social work or related field, and at least two years of case management

Qualifications

• Active Behavioral Health license in IL (LCSW/LCPC/LMFT)
• Must reside in Illinois near Zion, Waukegan, or the Lake County area
• Minimum of two years of case management experience
• Microsoft Office and electronic health record experience
• Must have reliable transportation and be willing to travel up to 75% of the time (mileage reimbursed)
• Master's degree in social work, counseling or related field

Responsibilities

• Conduct comprehensive in-person assessments and develop individualized care plans
• Coordinate physical, behavioral, and social services across providers
• Authorize services and document care activities
• Participate in interdisciplinary meetings and support team development
• Pursue ongoing professional growth while providing compassionate care
