Job summary

CVS Health Corporation is seeking a fully licensed clinical social worker to provide advanced clinical judgement in coordinating physical and behavioral health services. You will manage case load, coordinate with providers, and guide wraparound services to achieve optimal, cost-effective outcomes.

This full-time role requires strong communication, motivational interviewing, documentation, and the ability to travel up to 125 miles for field visits.

Qualifications

• Active clinical licensure (LCSW, LPC, LMFT) required.
• Case management experience.
• Strong customer service orientation and problem solving.
• Excellent communication and telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport telephonically.
• Excellence in documentation and regulatory compliance.

• Ability to travel up to 125 miles round trip for a 1-day field visit.
• Variable schedule with potential to work 2 days weekly until 9pm local time.
