WellPower in Denver is seeking a Clinical Case Manager I/II for the Forensic ACT program. You will provide psychosocial services across high and low intensities, transport clients with reimbursement for vehicle costs, and support court and community engagement to enhance recovery and functioning.

Qualifications include a CM I/II degree in psychology or social work, plus a Colorado driver’s license and court/community outreach experience.

Qualifikationen

• Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or other related human services degree. Licensure preferred (LPC, LCSW, LMFT, or Licensed Psychologist) or have continual committed hours towards licensure and are currently under WellPower clinical supervision.
• One year of experience working with individuals with severe and persistent mental health conditions or in human services.
• Must have valid Colorado Driver’s license and a recent clean driving record. Ability to transport the people we serve in your vehicle.

Aufgaben

• Provide comprehensive psychosocial services that include a continuum of high and low intensity services to improve or maintain individuals’ functioning.
• Attend court with clients; court appearance 1-2x/week.
• Maintain jail clearance and securus access to meet with individuals in custody.
• Conduct weekly outreach in courts, jails, shelters, hospitals, and the community; meet clients at jail releases and inpatient facilities.
• Arrange transportation or transport clients using personal vehicle (business insurance reimbursed).
• Identify and engage natural support systems and connect clients to resources and housing options.
• Support families, friends, landlords, employers, community agencies, and stakeholders to maximize support and reduce stigma.
