Job summary

CVS Health is hiring for a Case Management Coordinator in Ohio. The role is full-time and field-based with telework options within the state.

Travel to member homes and other locations is required up to 50% of the time, typically Monday–Friday, 8–5 with flexibility for extended hours as needed. Responsibilities include coordinating care plans, collaborating with multidisciplinary teams, and educating members to improve outcomes.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related public sector field.
• 2+ years in family systems, community resources, or case management.
• 2+ years with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio and travel up to 50%.

Responsibilities

• Coordinate and facilitate care plans; monitor progress and outcomes.
• Collaborate with case managers, supervisors, and health teams to resolve gaps.
• Provide education and coaching to empower members in healthcare decisions.
• Serve as a single point of contact for members to remediate gaps in care.
• Document care activities in line with regulatory and company policies.
