Qualifications

• 2+ years in behavioral health or related field.
• Willingness to travel 50%+ within central WV region; mileage reimbursed per policy.
• Proficient use of MS Office and multiple computer systems.

Responsibilities

• Evaluate referred member's needs/eligibility and plan care using care management tools.
• Identify high-risk factors and coordinate referrals to clinical case management or crisis interventions.
• Coordinate and monitor care plan activities and progress to meet member goals.
