Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.

Responsibilities

• Complete member assessments per regulated timelines to determine eligibility for care coordination/care management.
• Develop and implement care plans with member, caregiver, physician and other health care professionals and support network.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plan effectiveness, document interventions and goal achievement, and adjust as needed.
• Maintain ongoing member caseload with regular outreach and management.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing and Molina clinical guideposts to support and motivate change.
• Identify barriers to care and assist members in addressing concerns.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required depending on state/contracts.
