Qualifications

• 2+ years in health care, preferably care management or related setting.
• Clinical licensure and/or certification only if required by contract or regulation.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.
• Knowledge of community resources.
• Able to work proactively with strong detail orientation.
• Able to adapt to diverse settings and populations, with minimal supervision.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office and relevant software.

Responsibilities

• Provide support for care management and care coordination with a multidisciplinary team.
• Complete member assessments and determine eligibility for care coordination.
• Develop and implement care plans with member, caregivers, and health professionals.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor care plans, document interventions, and adjust as required.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Use motivational interviewing and Molina guidelines to educate and motivate change.
• Identify barriers to care and provide assistance to address concerns.
• Collaborate with licensed care managers as needed.
• Travel 25–40% locally as required by state/contractual needs.
