Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician (LCSW/LMSW or equivalent) active and unrestricted.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• Experience with severe mental health concerns and substance use disorders.
• Knowledge of whole-person care, chronic conditions, and discharge planning.
• Data entry skills and experience with a clinical platform.
• Excellent verbal and written communication skills.
• Microsoft Office suite proficiency.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines and identifies care coordination candidates.
• Develops and implements care plans with member, caregivers and providers.
• Conducts telephonic, in-person, or home visits as needed.
• Monitors progress, documents interventions, and adjusts plans.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS, and community resources.
