Job summary

Molina Healthcare is seeking an experienced RN Care Manager to support care management and coordinate services across the continuum. You will work with a multidisciplinary team to promote quality and cost‑effective member care and ensure progress toward expected outcomes.

Responsibilities include comprehensive member assessments, developing and implementing care plans, and conducting telephonic, in‑person or home visits. Travel 25‑40% may be required.

Qualifications

• At least 2 years experience in health care, care management, or related setting.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation.
• Familiarity with EMR and HIPAA.
• Strong knowledge of community resources.
• Ability to work independently with attention to detail.
• Excellent time management and prioritization.
• Strong verbal and written communication.
• Proficient with MS Office and navigating portals/databases.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with member and care team.
• Conduct telephonic, in-person or home visits as required.
• Monitor and document progress; adjust plan as needed.
• Maintain ongoing member caseload and outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate members using motivational interviewing.
• Address barriers to care and provide coordination assistance.
• May provide consultation to peers; assist with complex cases and medications.
• Travel 25–40% may be required.
