Job summary

Molina Healthcare provides support for care management and care coordination across the continuum, collaborating with a multidisciplinary team to ensure member progress toward outcomes and alignment with cost-effective care strategies.

Responsibilities include assessments, care plan development with the member and care network, and ongoing monitoring. Travel as required by state contracts supports comprehensive member care delivery.

Qualifications

• At least 2 years experience in health care, preferably care management, or equivalent combination of education and experience.
• Clinical licensure and/or certification required ONLY if mandated by state contract/regulation.
• Valid driver’s license, reliable transportation, and adequate auto insurance for travel requirements.
• Demonstrated knowledge of community resources.
• Ability to work proactively, with attention to detail.
• Ability to work across diverse settings and adjust style as needed.
• Ability to work independently with minimal supervision and self-motivation.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office or applicable software.
• In some states, a bachelor’s degree in a health care field may be required.

Responsibilities

• Provides support for care management and coordination activities with a multidisciplinary team.
• Develops and implements care plans with member, caregiver, physician and others.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and goals.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services to enhance continuity of care.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination assistance.
• Collaborates with licensed care managers as needed.
• Travel up to 25-40% may be required depending on state/contractual needs.
