Job summary

Molina Healthcare, Inc. is seeking a Care Coordination/Case Management professional to support behavioral health care delivery. You will collaborate with a multidisciplinary team to coordinate integrated member care across the continuum and strive to ensure progress toward outcomes.

Essential duties include conducting assessments, developing care plans, performing visits, monitoring progress, and promoting service integration. Role requires 25–40% local travel and strong communication skills.

Qualifications

• 2+ years health care experience, preferably in behavioral health.
• Active licensed behavioral health clinician (LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD/PsyD) per state requirements.
• Valid driver's license and reliable transportation for travel.
• Experience with severe mental health concerns and SED, including substance use and foster care.
• Knowledge of whole person care, chronic conditions, and discharge planning.
• Data entry on a clinical platform.
• Excellent verbal and written communication.
• MS Office or equivalent proficiency.

Responsibilities

• Completes comprehensive behavioral health assessments per regulated timelines and determines eligibility for care coordination/case management.
• Develops and implements care plans with member, care network and professionals to address needs and goals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan to evaluate effectiveness, documents interventions and goals.
• Maintains ongoing member caseload for outreach and management.
• Promotes integration of services across behavioral health, LTSS, home and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina clinical guideposts to support change.
• Assesses barriers to care and provides coordination and assistance.
• May provide consultation and recommendations to peers.
• Local travel 25-40% may be required.
