Job summary

Humana is seeking a Care Coordinator to support Illinois Medicaid Members within the MLTSS/LTSS programs. You will assess member needs, develop personalized care plans, and link individuals and families to resources to enhance health and well-being.

The role is field-based with 75-90% local travel, focusing on coordinating services, monitoring progress, and addressing barriers in the member's care journey while ensuring compliance with program requirements.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with 1 year experience conducting comprehensive assessments.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).
• Must reside in Illinois.
• 1+ year experience in health care and/or case management.
• 1+ year experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
• Intermediate to advanced computer skills: Word, Excel, Outlook.
• Experience using electronic medical records.

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and Long-Term Care Facilities with 75%-90% travel.
• Assess care needs and plan interventions to meet them.
• Coordinate services, monitor and evaluate the care plan.
• Assist enrollees with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Connect enrollees with community resources.
• Guide families toward resources for their care.
