Job summary

Humana iCare is seeking an RN Care Manager to join the Family Care Partnership (FCP) program in Wisconsin. You will perform nursing assessments, support adults in the community, and work with an IDT to ensure quality, cost-effective care.

Travel up to 50% across Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan counties. The role requires Wisconsin RN licensure and at least 2 years of health care experience, with BSN preferred.

Qualifications

• Must be a Registered Nurse licensed in Wisconsin.
• Two (2) years of related health care experience.
• Must be able to travel to counties Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan.

Responsibilities

• Complete nursing portion of the comprehensive assessment and provide input to the member's care.
• Monitor wellness and health promotion activities based on assessment and outcomes.
• Re-evaluate member's health and wellness at appropriate intervals.
• Perform routine health monitoring activities based on member's need.
• Build relationships with members, family members, and outside agency staff.
• In crisis situations, assess psychiatric/behavioral/medical problems and implement interventions.
• Evaluate members' ability to manage medications and treatments safely.
• Provide or arrange services and monitor interventions.
• Assist with program, policy, and procedure development and input for QA/QI.
