BCFS Health and Human Services in Tyler, Texas seeks a Resource Navigator to ensure quality services while complying with program standards. The role emphasizes sound judgment and best practices in serving families.

You will conduct outreach, intake, assessments, and develop goals with parents, provide intensive case management, coordinate resources, and document activities in PEIRS and Salesforce. Some travel may be required.

Qualifications

• High school diploma or equivalent required.
• 3+ years of related work experience with high school diploma.
• Bachelor's degree preferred; 1+ year related experience with Bachelor's.

Responsabilités

• Outreach and recruitment of participants for Family Resource Center activities.
• Establish and maintain a trusting and professional relationship with families.
• Administer intake, assessments, and screening tools to clients and family members.
• Use intake results to set goals, objectives, and activities with parents.
• Provide intensive case management services to families.
• Coordinate services, referrals, basic needs support, transportation assistance, and other support services.
• Document all services and activities in PEIRS (DFPS) and Salesforce (BCFS).
• Meet with supervisor bi-weekly to review status of families on caseload.
• Travel as needed.
• Other duties as assigned per grant requirements.
