CAMBA in East Elmhurst, NY seeks a Case Manager to support families with housing stability, benefits and social services. You will conduct intake, manage CARES records, coordinate with housing providers and community partners to help families achieve ILP goals and rehousing milestones.

Minimum qualifications include a Bachelor’s degree with 1 year of direct experience preferred, or an equivalent combination; MS Office proficiency and ability to obtain required clearances are essential.

Formación

• Bachelor's degree with 1 year of direct experience preferred or alternative combinations as listed.
• Proficiency in MS Office and basic office technology.
• Bilingual abilities are preferred; strong communication and confidentiality are essential.

Responsabilidades

• Establish and maintain professional relationships with families while protecting confidentiality.
• Enter and maintain client information, assessments, and case notes in CARES.
• Coordinate appointments with housing providers, government agencies, schools and other service providers.
• Advocate for families and liaise with outside agencies to address housing, benefits, and other needs.
• Complete intake, eligibility, and case management activities for assigned families.
• Prepare and review ILPs, progress notes, and program reports.
