Job summary

CAMBA is hiring a Case Manager in East Elmhurst, NY to coordinate housing and support services for families. The role involves building professional relationships, protecting client confidentiality, and delivering comprehensive case management within CAMBA’s DHS-funded programs.

Responsibilities include intake, eligibility reviews, CARES documentation, ILP development, and coordinating with housing providers and agencies to help families achieve stabilization and permanent housing.

Qualifications

Bachelor's Degree and one year of direct experience preferred OR
Associate's degree and three year of direct experience OR
High school diploma or equivalent and six years of direct experience; or applicants pursuing completion within next three months

Responsibilities

Establish professional relationships with families while protecting confidentiality.
Complete the intake process and review eligibility, family composition, benefits, income, and service needs.
Create and maintain accurate client case records and required documentation.
Enter and maintain client information, assessments, case notes, referrals, progress in CARES.
Conduct CARES assessments and updates to identify housing, employment, income, benefits, and needs.
Develop and update independent living plans (ILPs) with short- and long-term goals.
Coordinate appointments with housing providers, government agencies, healthcare providers, schools, and community organizations.
