Job summary

Integrated Services for Behavioral Health (ISBH) is a community-minded organization serving Southeast and Central Ohio with behavioral health services. We are seeking a Care Coordinator to work with children, youth and families, coordinating care in home, school and community settings.

The role involves engaging families, identifying needs, and developing collaborative care plans while ensuring safety, quality, and cultural humility.

Qualifications

• Experience providing services and/or support to children and families in behavioral health or related public sectors.
• Education pathways include high school diploma or higher, with degrees up to master’s level preferred.
• Knowledge and experience in Hi‑Fidelity Wraparound is preferred (Certification provided at time of employment).
• Two years of experience in a coordinated supportive services or care coordination role is preferred.
• Experience with autism spectrum disorders and developmental disabilities is preferred.

Responsibilities

• Identify and coordinate care coordination needs with families to meet service delivery standards and outcomes.
• Work with families to define cultural factors influencing engagement and plan effectiveness.
• Coordinate family-based services at home, school, and community.
• Ensure services align with safety and treatment needs and least restriction.
• Engage with child and family teams to support integration of plans.
