Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator to organize and lead a multidisciplinary team to support youth and families within the OhioRISE program. The role includes coordinating services in home, school, and community settings, maintaining detailed documentation, and ensuring HIPAA compliance.

The position requires experience in care coordination, strong communication, and the ability to travel within the service area.

Qualifications

• Experience providing services and support to children and families in behavioral health or related fields.
• Hi-Fidelity Wraparound experience preferred (certification provided)
• Two years of experience in a coordinated supportive services or care coordination role with high efficiency and attention to detail
• Experience working with autism spectrum disorders and developmental disabilities is preferred
• Experience in family systems, community resources, case management, or related fields is required
• Ability to bill Medicaid without restrictions and have a valid driver's license

Responsibilities

• Lead and facilitate team meetings for youth and family teams.
• Provide care coordination in homes, schools, or community settings.
• Identify care needs and services with the youth, family, and providers.
• Maintain documentation to access OhioRISE benefits and services.
• Travel within assigned service area as needed.
• Comply with HIPAA and required trainings and certifications.
• Utilize EHR systems for client documentation and reporting.
• Ensure confidentiality and professional conduct in all interactions.
