Integrated Services for Behavioral Health (ISBH) in Athens, OH, is seeking a Care Coordinator to support youth under OhioRISE. This role coordinates services in home, school, and community settings, leading teams that include families and providers to achieve optimal outcomes.

The position emphasizes voice and choice for families, documentation, and adherence to HIPAA. Travel within the service area and ongoing trainings are required.

Qualifikationen

• Experience working with children and families in behavioral health or related fields.
• Must have a valid driver’s license and access to a vehicle.
• Ability to maintain confidentiality and HIPAA compliance.
• Ability to meet deadlines in a fast-paced, high-volume environment.

Aufgaben

• Develop, lead, and facilitate team meetings for the young person and identified team.
• Provide care coordination in the family’s home, school, or community setting.
• Collaborate with families, natural supports, and service providers to identify needs and services.
• Maintain required documentation to access OhioRISE benefits.
• Travel within assigned service area and participate in trainings.
