Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator in Jackson County, OH to support youth and families. The role focuses on coordinating care across home, school, and community settings, leading team meetings, and ensuring voice and choice in services.

Ideal candidates have experience with children and families, strong communication skills, and the ability to travel within the service area. OhioRISE knowledge is a plus, with a comprehensive benefits package offered.

Qualificações

• Experience providing services to children and families in behavioral health or related public sectors.
• Knowledge of Hi-Fidelity Wraparound preferred (certification provided).
• Two years in a care coordination or similar role with high efficiency and attention to detail.
• Ability to work with families facing complex needs and multiple systems.

Responsabilidades

• Lead team meetings with the family and young person to identify goals.
• Provide care coordination in home, school, or community settings.
• Collaborate with service providers and natural supports for outcomes.
