Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordinator in Ohio to organize multi-agency teams supporting youth in home, school, and community settings. The role emphasizes family voice, coordination across providers, and travel within the assigned area.

Responsibilities include leading team meetings, maintaining HIPAA-compliant documentation, and ensuring access to OhioRISE benefits. Strong preferred experience with Wraparound and behavioral health services.

Qualifications

• Experience providing services to children and families in behavioral health settings.
• Hi-Fidelity Wraparound knowledge preferred (certification provided at time of employment).
• Two years in a care coordination or coordinated services role preferred.
• Experience with autism spectrum disorders and developmental disabilities preferred.

Responsibilities

• Lead and facilitate team meetings for the youth and family.”

• Collaborate with family, natural supports, and providers to meet needs.
• Maintain documentation to access OhioRISE benefits and services.
