Job summary

Integrated Services for Behavioral Health (ISBH) in Pickaway, OH is seeking a Care Coordinator to connect families with services and coordinate care across home, school, and community environments. • Experience providing services to children, youth and families in behavioral health or related public sector human services.
• Ability to engage with families to identify care coordination needs and develop service plans.
• Knowledge of community resources and collaboration across child and family services.

Responsibilities
